Meter Set Request

Please fill out all the information below clearly and precisely. This will insure prompt and accurate billing for establishment of services. Applicable fees including deposits, meter set, meter tap, and access fees are required to initiate service. Deposits are based on meter size and/or type of business conducted. The deposits are due at the time of application or can be billed at the discretion of The Utility Billing Office.
